ALW rate calculator: what the waiver pays a home

DHCS pays a participating home a daily rate for each care tier. Pick the tier and the days to see the month's arithmetic. The resident pays room and board separately, from income.

ALW pays the home for care, about
$4,042.29
$132.97 a day times 30.4 days. Paid by Medi-Cal to the home, not by the resident.
The resident pays the home for room and board
$1,444.07
The most a home may charge an SSI/SSP resident for basic services in 2026. The resident keeps at least $182.00.
ALW pays the Care Coordination Agency
$320.00
A month, per participant. No charge to the resident.

Arithmetic from the DHCS 2026 rate sheet and the CDSS 2026 SSI/SSP notice, rounded to the cent. Not a DHCS figure, not a quote and not a bill. DHCS sets the tier after a nurse assessment, and nobody can say in advance which tier a person will get.

How to read the result

The first figure is what Medi-Cal pays the home for care under the Assisted Living Waiver (ALW), for the days the person lives there as an enrolled participant. It is never billed to the resident. The second figure is what the resident pays the home for room and board. For a resident whose only income is SSI/SSP, California caps the charge for basic services at $1,444.07 a month in 2026. For a resident with other income, DHCS sets no cap, so ask the home for its rate in writing. The third figure is the monthly care coordination rate Medi-Cal pays the Care Coordination Agency (CCA), the state-listed organization that does the ALW paperwork and care planning with the resident.

What decides the tier

A registered nurse from the agency assesses how much help the person needs, day and night, and DHCS reviews the assessment. The tier comes from the person's needs, and it can be reviewed when needs change. No home or agency should promise a tier before the assessment. How the assessment works.
