For professionals
Refer a patient or client for the Assisted Living Waiver
Leave your own details and a coordinator calls you to take the referral. No patient information on an open form.
How the referral works
- 1Leave your details
Name, organization, role, work email and phone. That is all the form asks; patient details never go through it.
- 2A coordinator calls you
Within 24 hours, Monday to Friday, 10 a.m. to 5 p.m.. The referral is taken by phone or through a secure route, with the checklist below in hand.
- 3The person is contacted
Beta reaches the person or their representative, explains the waiver and, if they choose Beta, starts the waitlist request with them.
ALW cannot meet a discharge date. The state holds the waitlist and releases openings about once a month. No agency can predict the wait or move someone up. Keep a second discharge plan running. Hospital and SNF referrals: what to expect
