What working with Beta's care coordinators looks like

When a person enrolled in the Assisted Living Waiver (ALW) through Beta chooses your facility, your contact is that resident's Beta care coordinator. Before move-in, the coordinator shares the nurse assessment and the care plan with you, and you decide on admission. After move-in, the coordinator visits monthly and reassesses when needs change.

Important: Beta is a Care Coordination Agency (CCA): the state-listed organization that does the ALW assessment, care planning and enrollment paperwork with the participant. Working with Beta does not grant DHCS provider approval, guarantee referrals or create preferred placement. Beta never sells waitlist priority or steers a participant toward a facility because of a business relationship.

Before a placement

The participant chooses the facility. The participant records that choice on the DHCS Freedom of Choice form, which the care coordinator collects. When the choice is your facility, the coordinator shares the assessed needs and the proposed care plan (the Individualized Service Plan, or ISP). The participant authorizes that sharing. You tell the coordinator about your opening, your licensed scope and your residential terms. Then you decide whether you can provide this plan for this person.

The coordinator then works through the admission paperwork with you and the participant. That means the facility and physician forms, the ISP, the rights and choice forms, and your residential agreement. Your admission decision and DHCS enrollment stay separate. For a resident already in your facility, DHCS sets two deadlines. The Assessment Tool is submitted within 45 days of the nurse's signature, and the ISP within 10 days of the completed assessment.

After enrollment

Monthly visits. The coordinator visits every month, in person unless the participant chooses a video or phone visit. Have staff observations, incidents, medication changes, hospital use and any concerns ready. The participant must be able to raise concerns without pressure.

The care plan. You provide the ALW services assigned to your facility in the ISP, within your license, and document delivery. Tell the coordinator if the plan no longer matches the person.

Changes in condition and incidents. Call the coordinator when a resident's condition, needs, providers or safety change. The change-of-condition triggers list what to report. Report incidents under licensing, ALW and emergency rules first, then tell the coordinator.

Reassessment and tiers. The coordinator follows the DHCS reassessment and tier-change process; a tier increase is not guaranteed and is not a price negotiation. The DHCS Re-Assessment Checklist, Tier Change Checklist and ISP Desk Guide are on the DHCS provider resources page ↗.

Records. For resident records, call the coordinator and give professional context only; the coordinator names the route Beta uses. Never send resident records by personal text or ordinary email.
