What working with Beta's ALW care coordinators looks like, day to day.

What ALW facilities can expect from Beta's care coordinators before placement, at move-in and after enrollment, and the profile information Beta collects.

Once a resident is enrolled in California's Assisted Living Waiver (ALW) through Beta, your point of contact is the resident's Beta care coordinator. Before move-in, we share the assessment and care plan so you can judge the fit; the admission decision is yours. After move-in, we visit monthly and update the plan when needs change.

Important: Beta is a Care Coordination Agency (CCA), the state-listed organization that does the ALW assessment, care planning and enrollment paperwork with the participant. Working with Beta does not grant DHCS provider approval, guarantee referrals or create preferred placement. Beta never sells waitlist priority or steers a participant toward a facility because of a business relationship.

Before a placement

When a participant's needs point to your facility, the care coordinator shares authorized information, assessed care needs and the proposed care plan (the Individualized Service Plan, or ISP). You tell us about your opening, licensed scope and residential terms. The question is whether you can implement this plan for this person; the participant chooses, and you decide on admission.

Admission and move-in

The care coordinator works through the admission paperwork with you and the participant, facility and physician forms, the ISP and who is responsible for each service, rights and choice, and the residential agreement. Your admission decision and DHCS enrollment stay separate.

After enrollment

Monthly visits. The care coordinator visits every month; have staff observations, incidents, medication changes, hospital use and any concerns ready. The participant must be able to raise concerns without pressure.

The care plan. You provide the ALW services assigned to your facility in the ISP, within your license, and document delivery. Tell the coordinator if the plan no longer matches the person.

Changes in condition and incidents. Call the care coordinator promptly when a resident's condition, needs, providers or safety change, see the change-of-condition triggers. Report incidents immediately under licensing, ALW and emergency rules, then loop in the coordinator.

Reassessment and tiers. The coordinator follows the DHCS reassessment and tier-change process; a tier increase is not guaranteed and is not a price negotiation.

How we stay in touch. For each participant we agree contacts, an escalation path and a secure communication method; sensitive records never go by personal text or ordinary email.
