What this website keeps about you
What you type into a form, with the page you sent it from, your language and the time. It goes to Beta's ALW team. No cookie is set. If you sign up for a training course, the site also keeps your training record, described below. Page statistics are described in the next section.
Cookies and page statistics
This site sets no cookies. It counts page visits with Umami, open-source statistics software that Beta runs itself and serves from this site's own address, not from another company. A short notice says so on your first visit; the count does not wait for an answer. You can turn it off for your browser from that notice or under "Privacy choices" in the footer; the choice is kept in your browser's local storage, not in a cookie. A Global Privacy Control signal from your browser turns it off as well.
The count records the page, the page before it, your language, screen size, browser type and a rough location worked out from your network address, how fast the page loaded and responded on your device, plus which of the call, Apply for ALW, Request a callback, Refer a patient or client, Check whether ALW may fit and DHCS source links, and the links into Beta's ALW portal to apply, refer or sign in, get used, with the page each click came from. When a link to this site carries a campaign label, such as a link in a post on Beta's Facebook page, the count keeps that label (utm_source, utm_medium and utm_campaign) and drops the rest of the address after the page. Visits from one browser are grouped by a code that changes every week; the network address itself is not kept. Nothing you type is sent, and it stays off on the application, referral, callback, contact, coordination and shortlist pages.
The map on the homes directory and on each home's page loads its map images from OpenStreetMap, which sees your network address and the map area shown. OpenStreetMap's privacy policy applies to those images. Nothing else on this site loads from another company.
The homes directory writes your search words and filters into the page address, so you can bookmark or share a search. A shared address carries those words with it. Homes you save to a shortlist stay in your browser until you send the list to Beta.
Forms on this site
Every form sends what you type to Beta's ALW team, so a person can call or reply. What each form asks for:
- Request a callback: your name, phone number, your question and the best time to call.
- Send a message: your name, email, your question and a phone number if you add one.
- Refer a patient or client, first screen: your name, organization, role, work email and a phone number. No patient details.
- Ask Beta to check this home: your name, phone number and the home's name, plus a short note if you add one.
- Send my shortlist to Beta: your name, phone number and the homes on your list. Beta creates a link that opens the same list, and anyone with the link can open it.
- Contact the ALW team, and Coordinate care with the ALW team: your name, facility or organization, role, phone, work email, county and the topic. A note is optional. When a home claims its profile, its CDSS facility number too.
- Report something wrong on this page: your note, and an email if you want a reply.
With each request the site also keeps the page you sent it from, your language and the date and time. It also keeps the version of the consent text you saw. It keeps no browser details and no network address. To block floods of requests, the server counts requests per network address and per phone number or email for 10 minutes, in memory only.
Beta stores requests in its database and passes them to its ALW team. A callback or message is not an application and does not put anyone on the state waitlist. Please do not include medical details, Medi-Cal numbers or Social Security numbers.
No form on this site asks for health, Medi-Cal or identity details. Those are asked only by a coordinator on the phone or in Beta's application form, and only what the ALW waitlist request needs. That information is protected health information and is handled under Beta's Notice of Privacy Practices.
The application form
"Apply for ALW" opens Beta's application form, hosted by PandaDoc outside this website. What you enter there goes to Beta's ALW team. PandaDoc's privacy notice applies on that page. This website does not see what you type there.
Training courses
You can read every lesson of the courses under Training without signing up. Until you sign in, your progress stays in your browser only. To take a course check and get a completion record, you sign in with your email. The first time, you also give your name, your role and, if you choose, the facility you work for. Beta's sign-in service emails you a one-time code and a link; there is no password. Your browser keeps the sign-in until you sign out.
While you take a course the site keeps, under your sign-up, which lessons you finished, each answer you chose and when, and your course-check results. When you pass a course the site records the completion with a verification code. Anyone who has that code can confirm the course, its version, the date, the score and the name and facility you gave at sign-up; nothing else, and nothing without the code. If you named a facility, that facility can see how many people who named it finished each course, never who.
The training record is kept so your progress follows you across devices and so a completion can be confirmed later. It is not health information and no resident or participant detail is asked for. You can correct your name, role and facility under My training. To remove your record, sign in and contact the ALW team at the number or email below.
If you use the renewal checklist under My training, the site also keeps the continuing-education courses you type into it (who gave each one, its title, date, hours, whether it was self-paced, its topic and, if you add it, the certificate number) and the date your administrator certificate expires. Only you can see them. They are not sent to CDSS or anyone else.
If your employer uses Beta’s training and you accept its invitation or ask to join with its home code, you first see exactly what it will see: your name and sign-in email, the courses it assigns you, your progress and completions on those, and any other completion you choose to share. It never sees your answers, your notes or your renewal checklist. You can leave at any time under My training; when you leave, your employer keeps a read-only copy of your completions on the courses it assigned, for licensing audits, and your own record stays yours. A manager’s invitation keeps the name, email, job and start date the manager typed until it is accepted, cancelled or expires.
Text messages and emails
This website sends no text messages today. Besides the training sign-in code and link described above, this site emails a training invitation when your employer’s manager asks it to, and reminders about courses your employer assigned you (seven days and one day before the due date, and one and seven days after, until you finish). The only other email this site causes to be sent is the training sign-in code and link described above; the sign-in code and link for referrals are switched off. Beta's ALW team answers a form by phone or by email, as the form says. A form's "Text-message terms" link brings you here. We do not sell or share your mobile number or text-message opt-in information with third parties for marketing. We do not use it for marketing at all.
