Assisted Living Waiver by county
ALW runs in fifteen California counties and Beta coordinates care in all of them. Pick your county for local offices, homes on the list and how to start.
The Assisted Living Waiver (ALW) runs in fifteen California counties: Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara and Sonoma. Beta's ALW team coordinates ALW care in all fifteen. Each county page gives the county offices to call, where to look for a home and how to start.
Beta is listed by the California Department of Health Care Services (DHCS) as a participating Care Coordination Agency under the name Beta Hospice Care. Beta's ALW care coordination is separate from any hospice service.
Important: A county page gives the county offices to call, the first steps and the homes on the state's ALW list for that county. It cannot tell you whether your family member is eligible or when the state will release an opening (the state calls it a slot). Nor can it say whether a home has an open bed, will accept your family member or can meet their needs. Those answers come only through the official steps.
Choose your county
Each county list shows the homes on the state's ALW list, checked on the date shown. A listing is not an open bed, and it does not say whether a home will accept your family member or can meet their needs. How to search and compare homes explains the directory.
Moving from another county
Your family member does not have to live in an ALW county today. To enroll, a person only has to be willing to live in one. You can read about ALW and take the fit check now. The eligibility page has the full rules. If the person lives in one of the 43 counties without the waiver, the options outside the waiver counties lists the nearest waiver counties and the programs that run where they are.
Before a move, think about who will visit, which doctors they would keep and how they will get to appointments. Ask whether a home can meet the needs the nurse assessment finds.
Choosing an agency
You may choose any Care Coordination Agency on the DHCS list of participating agencies. Beta is one of them. The agency sends the waitlist request to DHCS, does the nurse assessment and writes the care plan. Read what a Care Coordination Agency does.
If you're referring a patient or client
Call Beta's ALW team at (747) 293-5777. Give your organization, your role, a callback number, the person's setting and county, and the general need. Do not send patient details by email. The referral checklist says what to have ready. Refer a patient or client takes your own details, and a coordinator calls you back.

County pages are planning resources. DHCS decides eligibility after the nurse assessment and releases openings. Each home decides its own admissions. Nothing here is an eligibility decision or a promise of enrollment, and it does not guarantee a place in a home. You may choose any Care Coordination Agency listed by DHCS and any participating home.
