How to report an error
Email info@alwbeta.com. Please tell us:
- the page address,
- what looks wrong, and
- the official source, if you have one.
Please do not send medical details or ID numbers.
On a home's page, you can also use the "Report something wrong" link. On a training page, use the error link on that page.
What happens next
We check the report against the official source. If the page is wrong, we fix it and add a line to the log below. We cannot promise when a fix will be made.

This log began on November 4, 2026.
- November 4, 2026, Facts about Beta. The page said Beta Hospice Care, Inc. was the "parent company" of Beta ALW Care Coordination. They are one company; the row now says "Legal entity". The page also said the office was "the address DHCS lists"; DHCS shows the street address without the city or ZIP, so the line now says DHCS lists the same street address. Found in Beta's own review the day the page was published.
- November 4, 2026, For reporters. Added the note that the DHCS list of agencies shows Beta under six of the fifteen counties, which the facts page already carried.
