1. The state lists us
DHCS publishes the list of Care Coordination Agencies it has enrolled for the Assisted Living Waiver, by county. Beta appears on it under the name Beta Hospice Care. Search the DHCS website for its Care Coordination Agency list and find the name yourself. Beta now coordinates ALW care in all fifteen counties where the waiver operates; the DHCS entry has not been updated yet and still shows the original six (checked 2026-09-18).
2. The legal name and the office
The legal entity is Beta Hospice Care, Inc., established in 2020, with its office at 1525 N D Street, Suite 10, San Bernardino, CA 92405, open Monday to Friday, 10 a.m. to 5 p.m. (published on betahospice.com, read 2026-09-18). The name is the hospice line's, which is how the state listed the organisation; ALW care coordination is a separate line of work with its own team and phone number. The hospice line is licensed by the California Department of Public Health (licence 550004949) and Medicare certified since 2020 (CCN A11527) (CDPH and Medicare records, read 2026-09-18).
3. The people
The program pages are written by Beta's ALW program manager and reviewed by a registered nurse on the team before publication. Names are not published on this site yet; ask on the phone and the coordinator will tell you who reviewed a page.
4. What we can never do, and what it costs you
Beta does not decide eligibility, does not release openings, cannot move anyone up the waitlist and cannot promise a room. Anyone who tells you otherwise, in any agency's name, is wrong. Beta does not charge you for care coordination: Medi-Cal pays Beta directly, and no one at Beta may accept a payment or a gift for a referral or a placement.
5. Your choice, and where to complain
You may choose any Care Coordination Agency on the DHCS list and any participating facility, and you can change agencies through the state's process at any time. How to change your agency.
