How to change your care coordination agency

Change your ALW agency with the DHCS 0061 form, which says your benefits will not be altered or interrupted. The four steps and what transfers.

Yes, you can change your Assisted Living Waiver (ALW) Care Coordination Agency (CCA). The current state form says your ALW benefits will not be altered or interrupted because of the switch itself. You sign one state form, and the new agency sends it on. The change takes effect on the date you write on the form.

Important: If the problem you're switching over is urgent, such as a missed service or a safety issue, tell your current agency now. Do not wait for the transfer. The current state form says the switch itself will not alter or interrupt your benefits. If a monthly visit or a service is missed after the change, tell the new agency right away. Records move only with the proper privacy and authorization steps.

Four steps

  1. Pick a new agency that serves your county. The California Department of Health Care Services (DHCS) keeps the list of participating agencies. Your current agency or Beta's ALW team can tell you which ones serve your county.
  2. Call the new agency. Ask whether it can take you on and how it handles transfers.
  3. Sign the state's Change of Care Coordination Agency form, DHCS 0061. Open the DHCS 0061 form (PDF). Write the date you want the change to take effect. That "as of" date is the effective date. The new agency can help you fill it in.
  4. Keep a copy of the signed form. The new agency sends copies to your home, to the old agency and to DHCS. Ask the new agency to confirm when it has done so.

What the two agencies hand over

From the date on your form, your monthly visit and your records move to the new agency. You don't have to gather these yourself, but it's worth asking whether they've all moved.

  • Your care plan, which the state calls the Individualized Service Plan (ISP), and your most recent assessment or reassessment.
  • The services arranged for you and who provides them.
  • Your monthly-visit notes and any open concern, incident or change in your health.
  • Your contacts, your representative's authority, and the contacts for the home and your providers.

Your monthly visit continues with the new agency, by telehealth if you choose.

If you're moving as well

A move raises extra questions.

  • Is the new county an ALW county?
  • Does the new agency serve it?
  • Does the new home take ALW, and can it meet your needs?
  • Will your care plan need updating?

See homes on the state list, see ALW by county, and ask the new agency who coordinates the move date.

If you're switching because something went wrong

Changing agencies doesn't stop you from making a complaint about your previous coordination. Keep your documents, dates and what you'd like done. See your rights and how to complain.

Switching to Beta?

Tell Beta's ALW team the person's name, current county and home, current agency, why you'd like to switch, and anything urgent about continuity. Please don't send medical records by ordinary email or through a web form. If Beta needs records, your coordinator names the way to send them on the call.
