Which phone number for the Assisted Living Waiver?
For Beta's ALW care coordination, (747) 293-5777. For another agency, the state's list has each one's number: care coordination agencies by county. The Department of Health Care Services does not publish a phone line for the program: its Assisted Living Waiver page sends questions about services to your agency and concerns about an agency's coordination to ALWGrievances@dhcs.ca.gov.

Do not attach medical records, identification numbers or case documents. A coordinator tells you how to send records when they are needed.
Who you are
- Adult who needs ALW, or a family member: Apply for ALW. Applying asks Beta to be your Care Coordination Agency. The state decides eligibility.
- Hospital, nursing facility or community professional: Refer a patient or client.
- Assisted-living facility: Contact the ALW team.
- Hospice, home health, palliative or health plan team: Coordinate care with the ALW team.
If someone is unsafe right now, call 911 or Adult Protective Services in your county.
