Assisted Living Waiver

Every Care Coordination Agency on the state's ALW list

Every Care Coordination Agency on the DHCS Assisted Living Waiver list as read on October 12, 2026, with the counties each one lists, how to choose one and how to change later.

The California Department of Health Care Services (DHCS) lists the agencies a person may choose for the Assisted Living Waiver (ALW). A Care Coordination Agency (CCA) is the state-listed organization that does the ALW paperwork and care planning with you. The table below is DHCS's list as we read it on October 12, 2026, with the counties DHCS shows for each agency. You may choose any agency that lists your county, and you can change later with the DHCS change-of-agency form. Every agency sends waitlist requests to the same DHCS queue. No agency can move anyone up the list.

Important: DHCS keeps this list and changes it. Check the DHCS list of participating agencies before you rely on a county or phone number here. Being listed means DHCS has enrolled the agency to coordinate ALW care. It is not a rating, and Beta does not rank agencies. Beta is one of the agencies in the table. DHCS decides eligibility and releases openings (the state calls them slots) in priority order, when it releases capacity. No agency, including Beta, can promise eligibility, an opening, a date or a placement.

How to choose an agency

Every agency has the same job, because DHCS sets it. The agency screens your waitlist request within seven calendar days and confirms it in writing within three more. It sends the request to DHCS. When your opening is released, a registered nurse from the agency does the assessment, the agency writes your care plan with you and helps you compare homes, and it sends the enrollment package to DHCS. After you move in, your care coordinator visits every month and checks your Medi-Cal each month. What the agency does at each stage.

What differs is how the agency works with you: how quickly someone answers the phone, which languages the staff speak, how they keep in touch while you wait, and whether visits are in person or by telehealth. Ask each agency you consider:

  • Which counties do you serve, and who answers when I call?
  • How will I know where my request stands?
  • What languages does your intake team speak?
  • How do you help me compare homes while leaving the choice to me?
  • What do I do if I have a complaint?

Beta does not rank agencies. Don't choose one because it promises a shorter wait or a particular home. No agency can, and the promise itself is a warning sign.

How to change your agency

You can switch at any time with the DHCS Change of Care Coordination Agency form, DHCS 0061. The new agency helps you fill it in and sends it on. The change takes effect on the date you write on the form, and the form says your ALW benefits will not be altered or interrupted by the switch itself. Your services continue. How to change your agency.

Beta's DHCS entry shows six counties today. Beta serves all fifteen, so the Kern, Los Angeles, Orange, Riverside, San Bernardino and San Diego lists above name Beta Hospice Care as DHCS does.

Which Care Coordination Agency should I choose?

No. Every agency sends waitlist requests to the same DHCS queue, and DHCS releases openings in priority order. Choose the agency that lists your county, answers you and explains things clearly.

Does it cost anything to use an agency?

No. There is no charge to the person. Medi-Cal pays the agency a care coordination rate, $320.00 per participant per month in 2026. See the ALW rates.

Can an agency get me in faster?

No. An agency can do its own part promptly: screen within seven days, confirm in writing and have your documents ready when an opening is released. No agency can move anyone up the list or make DHCS release an opening sooner. How the waitlist works.

This page gives general California ALW information. DHCS decides eligibility after the nurse assessment and releases openings. Each home decides its own admissions. Nothing here is an eligibility decision or a promise of enrollment, and it does not guarantee a place in a home.
