What the list tells you, and what it does not

A name on the list means DHCS lists that organization as an ALW Care Coordination Agency, the organization that does the ALW paperwork, the nurse assessment and the care plan with you. It does not mean DHCS recommends it, and it says nothing about how the agency works with you. You may choose any agency on the list and change it later with the DHCS form.
