The ALW waitlist and enrollment timing for care coordinators

Five lessons for Care Coordination Agency staff: how the Assisted Living Waiver (ALW) waitlist works as DHCS describes it, the slot release order and priority groups, the CCA's work from a released slot to enrollment, timing the move with the home and the participant, and keeping people informed without promising what no one controls.

Before you begin

What you will be able to do

  • Given a new waitlist request, screen it, submit it to DHCS and send the written notice within the Policy Letter 24-002 times, and record the placement date.
  • Given the requests on a CCA's list, sort each into its priority group, file the proof the first group needs, and explain the release order without quoting a split or a date.
  • Given a released slot, plan the contact attempts, the assessment, the home's appraisal, the move and a complete application inside the 60-day window.
  • Given a participant's or a family's question, state the stage, the timing and room and board in plain words, and correct any promise of a slot, a date, eligibility or a room.
