Monthly monitoring, reassessment and tier changes

Five short lessons for CCA care coordinators and nurses on keeping an ALW plan current: the monthly contact the waiver requires, what to look for and record at each visit, the six-month reassessment and its packet, when a change in condition calls for an earlier reassessment, and how a tier change is assembled, decided and shared with the home and the participant.

Before you begin

What you will be able to do

  • Plan each participant's monthly contact so it meets the waiver: with the participant or responsible parties, in person or by a telehealth visit the participant chose, and recorded in the ISP's monitoring section.
  • Run a monthly visit that checks every ISP service, finds new risks, verifies Medi-Cal and reports abuse, neglect or other problems at once to the right agencies.
  • Keep each reassessment on schedule: at least every six months, inside the allowed window, with the full DHCS re-assessment packet and an updated ISP.
  • Decide when a change in condition or a request calls for an earlier reassessment or an ISP review, and leave the tier and service decisions to the Assessment Tool and DHCS.
  • Assemble a complete tier change packet on time, collect the signatures, and close the loop with the home and the participant once the plan changes.
