What a care coordination agency does
Every agency has the same job, because DHCS sets it. The agency screens your waitlist request within seven calendar days and confirms it in writing within three more. It sends the request to DHCS. When your opening is released, a registered nurse from the agency does the assessment, the agency writes your care plan with you and helps you compare homes, and it sends the enrollment package to DHCS. After you move in, your care coordinator visits every month and checks your Medi-Cal each month. What the agency does at each stage.
How to choose one, without a ranking
What differs is how the agency works with you: how quickly someone answers the phone, which languages the staff speak, how they keep in touch while you wait, and whether visits are in person or by telehealth. Ask each agency you consider:
- Which counties do you serve, and who answers when I call?
- How will I know where my request stands?
- What languages does your intake team speak?
- How do you help me compare homes while leaving the choice to me?
- What do I do if I have a complaint?
Beta does not rank agencies. Don't choose one because it promises a shorter wait or a particular home. No agency can, and the promise itself is a warning sign. Every agency sends waitlist requests to the same DHCS queue, so the agency you choose does not change your place on it.
If you already have an agency
You can switch at any time with the DHCS Change of Care Coordination Agency form, DHCS 0061. The new agency helps you fill it in and sends it on. The change takes effect on the date you write on the form, and the form says your ALW benefits will not be altered or interrupted by the switch itself. Your services continue. How to change your agency.
